I've received a few rejections from insurances when billing 71046 with an E/M. One in particular was billed with a 99395. There is a modifier 25 on the 99395 because we also billed out an 81002. Any suggestions?
We're also having an issue getting paid for this CPT code, specifically during annual exams. Aetna says we're using the wrong cpt code but what other chest xray cpt code could possibly be used instead??
