Wiki 69210 bundling with E/M or HT

We are having quite a lot of trouble getting our 69210 codes paid when billing with a hearing test and/or EM code. This has been pretty consistent lately with BCBS and now with UHC.
Has anyone else started seeing more of this or do you have any more appropriate ways to bill removal impacted cerumen requiring instrumentation (both single side and bilateral)?

You should be able to find the policy that states they are not separately reimbursed.
