When coding CPT 64615 for 150 units do you code for 1 unit or 150 units?

J0585 x 150 (I'm assuming the toxin is Botox).

Only other thing to consider is the total number of units in the vials that were used. If 2 100-unit vials were opened, but only 150 used, and the waste is unable to be used on any other patient, and units wasted is documented appropriately, you can bill for the full 200 units. Some payers require modifier JW to indicate waste--check their guidelines.

For example: pt comes in for Botox injection for Migraine headache. Injection code is 64615 and Botox code is J0585. 150 units of Botox were used with 50 units waste

We billed 64615 x 1 and J0585 x 200 units used. The waste cannot be reused so we bill for the full 200 units which is within the CMS guidelines and most carriers.

Beginning in January 2017, you will have to start using the JW modifier for the waste if billing Medicare.
