STATEMENT OF MEDICAL NECESSITY:
Mr. XX is a 74-year-old white male with increasingly
symptomatic right hydrocele. He was therefore consented to
right hydrocelectomy. The inherent risks of the operation
were outlined and he signed informed consent.

FINDINGS AND TECHNIQUE:
After consent was obtained, the patient was taken to the
operating room, placed in the supine position. After adequate
general anesthesia was established, the patient was shaved,
prepped and draped in the usual sterile manner. The skin
incision was made along the median raphe and the underlying
fascial layers were opened with the Bovie electrocautery. The
large hydrocele was delivered through the incision and a
tunica vaginalis was opened to evacuate a fair amount of clear
fluid. It was then everted upon itself in a standard bottle
neck closure using 3-0 Vicryl suture. After achieving
hemostasis with the Bovie electrocautery, the testis was
returned to its orthotopic position. The overlying dartos
layer was closed with a running 3-0 Vicryl suture and after
injecting 1% lidocaine with epinephrine as a topical
anesthetic, the incision was closed with a running 3-0 chromic
suture in a running horizontal mattress fashion. Sterile
dressings were applied consisting of Telfa, Tegaderm, fluffs
and a scrotal support and the patient was awakened from
anesthesia, taken to recovery in stable condition.

MEDICATIONS:
Norco 5/325 one to two every 4-6 hours p.r.n. pain, #30, no
refills.

He is instructed to remove the dressing in 48 hours and apply
Neosporin or triple antibiotic ointment daily for 7 days. I
will see him back in 2-3 weeks for a wound check.
 
55040 is getting denied for modifier for assistant surgeon AS. If there is an assistant surgeon, is there a modifier that can be used for the assistant?

In the operative report there was no indication of an excision of the hydrocele sac. I believe CPT code 55060 would be a better choice of code for what was done.
