Our doctor performed ESWL of a right urethral stone , which did not disintegrate well, so he performed a ureteroscopy with laser lithotripsy. This broke up the stone better, but some of the fragments advanced back up into the mid portion of the kidney. Because of the significant stone burden, decision was made to perform a repeat ESWL on the right kidney. Then there was complete destruction.

Can I bill 50590 twice? or how do I code this?

any help is greatly appreciated.

Modifiers....one of my favorites second only to E/M :)

Modifier 76 would indeed be correct if all of the services were performed on the same date. I am assuming, although I could be wrong, that these services were performed on different date in which case 76 would not be appropriate and 78 would fit better. Here are a couple of fact sheets from WPS, our MAC in Michigan that should help you decide which works best based on what was performed and when it was done.
