I understand that for 5010 format which becomes effective 01/01/2012 we cannot use PO Box addresses. If the pay to address is a PO Box address can we use the physical address for the provider or group in Box 33 of the HCFA 1500 form?

5010 is changing the way electronic claims are being sent. Your clearinghouse/vendors should help you to make sure that your physical and post office box is sent in the correct loops. As far as I know there are no changes to the actual paper CMS 1500 form. If you hear different please let me know.
