I understand that for 5010 format which becomes effective 01/01/2012 we cannot use PO Box addresses. If the pay to address is a PO Box address can we use the physical address for the provider or group in Box 33 of the HCFA 1500 form?:confused:
 
5010 is changing the way electronic claims are being sent. Your clearinghouse/vendors should help you to make sure that your physical and post office box is sent in the correct loops. As far as I know there are no changes to the actual paper CMS 1500 form. If you hear different please let me know.
