I know we cannot bill 45382 with 45380 if finding were in the same area but I need guidance if it is ok to bill them together if they are separate areas? Can I bill this as 45382 -59, 45385, AND 45380 -59?
The OP note reads-
(Any feedback is appreciated, thank you)
A single sessile 5 mm polyp of benign appearance was found in the proximal ascending colon. A single-piece polypectomy was performed using a cold snare.
A single sessile 3 mm polyp of benign appearance was found in the hepatic flexure. A piece-meal polypectomy was performed using a cold forceps
Two sessile polyps of benign appearance ranging in size from 3 mm to 5 mm were found in the transverse colon. Single-piece polypectomies were performed using a cold snare and cold biopsy forceps. The polyps were completely removed.
A single pedunculated 20 mm polyp of benign appearance was found in the sigmoid colon. A single-piece polypectomy was performed using a hot snare. The polyp was completely removed. 2 ml epinephrine 1/10000 injection was successfully applied for hemostasis. Two endoclips were successfully applied to the sigmoid colon for the purpose of hemostasis.
