Having a little bit of confusion on proper documentation for 3D rendering/reconstruction. Here are various examples of dictation and looking to correctly determine if the docs are supporting the 3D rendering - what seemed clear at one point is now causing some questions...Hopefully someone can assist in confirming.

Does the following constitute supportive documentation to bill 3D?

Axial images of ct of the head was performed. Sagittal and coronal reconstruction images were performed.

Axial volume acquisition of ct abdomen and pelvis was performed. 3D processing was done from lung base to lower pelvis.

ct facial bones was performed with transaxial sections with coronal and sagittal reconstructed images also obtained.

Axial images of the sinuses were obtained. Sagittal and coronal reformatted images are available for review.

Axial and sagittal images were obtained. Axial and sagittal STIR images were also obtained. Coronal STIR images were obtained for MRI of lower leg.

Ct scans of the left shoulder were performed with sagittal and coronal reconstructions. It later goes on to say that mass is seen on main axial images of scan performed.

Any help to ensure this is appreciated. Referenced several sites for proper documentation requirements but looking for some other expert opinions :)
