Hello coding community,
Could you please help me clarify when I should use modifier 50 with 38900? I see a lot of Op notes where a surgeon injects the dye into a cervix bilateral/both sides. Does it qualify for mod 50? I will place some examples of the notes below. I checked that mod 50 is allowed and per oncology file, they recommend to use mod 50 but they do not specify into what anatomical area the dye is injected. Again, all our OP notes show the injection into cervix but cervix does not have laterality. What should we do? And does mod 50 increase the reimbursment? Thank you very much for your help.

Yes, this is absolutely justification for -50. The ICG dye is injected on each side of the cervix so that the mapping itself (represented by 38900) is bilateral.

How do you code for laparoscopic/robotic sentinel lymph node biopsy(ies) in endometrial and cervical carcinoma?
The code depends on dye injected. For non-radioactive dye use 38900, with modifier 50 if bilateral mapping is performed. In addition, bill the appropriate laparoscopic lymph node sampling or lymphadenectomy code depending on the extent and location of the dissection. If lymph node dissection is done because of non-mapping or other indications such as lymph adenectomy then the 38900-50 can still be billed with the lymphadenectomy codes.
