10021 is fna (fine needle aspiration) biopsy of a lesion.
The 38505 is for needle biopsy of a lymph node, not a 'lesion.'

I have seen documentation in an EHR/ Med Rec (2019). that shows 38505 as a fine needle aspiration biopsy of a lymph node. When I saw that documentation I became confused because in the cpt code book, it states for needle aspiration go to 10021 etc. Am I right in thinking the description in the Med Rec should not state 38505 along with the words (FNA)? Then I was thinking is 38505 is possibly a core biopsy? I guess I need more clarification regarding the biopsies. I thank you for your response.
 
That is what happens when the CPT codes are improperly loaded into the EHR and as a result, the doctors are lead to choose the wrong CPT codes. I would check the documentation to make sure the correct code is submitted.
