So I'm doing some hospital subsequent care audits for my practice: Code 99233. I see that with the 97 guildlines 3 chronic conditions can be listed for a detailed History. Can this be found in any part of the note?, my doctors arent stating it in the "history" but i am seeing it in the assesment and plan.
 
Dont quote me on this, however I have researched this very subject as well, and in some of these past AAPC threads it is mentioned that certain medicare carriers are allowing it to be pulled from any part of the note. My docs are doing the same,and I have found that if it is not a part of their assessment of that visit (for example pt inhouse for CHF exacerbation, but impression and plan states CAD stable) I am counting that as part of the chronic conditions. I would say overall, check with your local Medicare carrier for a more accurate answer.
