I have billed sclero for spider veins in legs but never for the scrotum. This might fall under "cosmetic" and not be a covered benefit. Each carrier has their own policy on what would fall under medical necessity.
It's hard to give good coding advice with limited information, but I'll give it a shot. If this is being done for some form of congenital or acquired malformation and the solution is being injected into a vein or multiple veins, I would suggest CPT 37241. This code has no MUE's, but should only be reported once per surgical field. If that code doesn't quite fit the situation, you may have to bill an unlisted code for this procedure. Hope this helps!
