Wiki 29881 with 29874, MPFL surgery

Hi fellow coders - -

Just a couple of coding issues that keep recurring - -

It is my understanding that it ok to bill removal of loose bodies in the knee 29874 with a meniscectomy if 2 or more loose bodies are removed AND they are greater than 5mm. We are using the modifier 59 on 29874. Is this accurate? I am submitting and appealing and still not getting paid.

Secondly, recently we have had a lot of MPFL reconstructions (arthroscopically assisted). Would you use 27422 and use a modifier? It seems like he does just a diagnostic arthroscopy and then does the MPFL open. The docs think if I use 29999 it is a better option. I am not so sure.
