Can anyone advise me when it would be appropriate to add a 59 modifier to 20610 for it to be payable?

That is a bit of a broad question. For example, if you are saying 20610 is being done at the same joint w/ 29825 that is very different from 20610 being done at the hip while 29825 is being done at the shoulder. I would not report 20610 w/ 59 separately if it was being done in the shoulder joint along w/ 29825.
