Hello,

I am hoping someone with orthopedic foot coding experience might be able to guide me. I have a provider that wants to use CPT code 27691 for tendon transfer of 2nd toe (FDL to extensor tendon) but isn't CPT 27691 for leg and ankle? The doctor is going through the foot, plantarly. She is also doing a hammertoe correction and capsulotomy on the same toe.

DX: long second metatarsal w/ crossover toe and hammertoe deformity

Why is the tendon transfer being done?

I'm fairly new to foot & ankle coding since we had a new doc join the group. I have not seen this situation...yet.... but looking at the Code-X, tenotomy and extensor tendon REPAIR is included with the hammertoe correction.

Your doc is performing a tendon transfer to the hind or mid-foot.

I would want to find out why the tendon is being transferred. Is it helping the hammertoe? If it is, you may have a hard time getting it paid. Unless it was done for a separate diagnosis, insurance companies will probably not pay for the procedure from what I can tell.
