Dr performs a Total Hip (27130). After it's completed he sees that the posterior aspect of the greater trochanter is still somewhat fragmented. He decides to perform a repair of this fragmentation (27248). Now would this revelation allow an addition of a 59 modifier since Medicare bundles 27248 with 27130. I think not since there was no separate incision but would it fall under a separate issue? Just want a second opinion.
