Our provider would like to bill for 20610, 77002 along with 27093 and I do not think the note supports this procedure. I think it was 20610 and 77002 Does anyone have an opioion on this?

from the op report:
The patient
was placed in a supine position, draped and prepped in the usual sterile fashion. A fluoroscopy machine was used to isolate the bony anatomy and injection site of the superolateral aspect of the femoral head, The overlaying skin was anesthetized using 1% lidocaine. A 25 gauge 3 1/2 spinal needle was serially advanced under fluoroscopy to conatact os at the mid-lateral aspect of the junction between the femoral head and femoral neck. The needle was redirected into the middle third fo the joint space. Contrast was injected revealing intra-articular and capsular spread, After negative aspiration, a 5ml solution containing 1 ml of 40mg/ml kenalog, 2ml lidocaine, and 2ml bupivicaine was injected. all injected medications were preservative free. the needle was then flused and withdrawn. there were not immidiate complications.

27093 is an injection procedure for hip arthrography without anesthesia. He does document that he injected contrast material and the results. However 20610 is a column 2 code for 27093 in the NCCI edits. A modifier is allowed but in the above note it seems that he used the same needle so it would not be appropriate to bill the 20610, just the 77002 and the 27093.
