We did an fusion/instrumentation at L2-3. Two months later the patient developes an osteoporotic fracture and we do a fusion/instrumentation at L1 to L3. We removed all the instrumentation and replaced with new. Should we use 22849 or 22842?

Just a quick thought...wouldnt use 22749 since you are doing a extra vertabrae and 22842 may be doable but that is a addition code to the primary procedure
