Hello,

Physician billed 20694-78 with dx 814.00 - claim denied as bundled and modifier
I'm changing the modifier to 58, and was wondering if the dx needs to be a follow up fracture code?? maybe V54.89

Why was the ex fix removed, was there a problem, normal healing, heald fx???? That depnends on the modifier and diagnosis code to use.

Probably needs to be a V52.x or V53.x for attention orthopedic device but definitely should not be the 814.00.
