I would use 20610-RT & 20610-LT

I would use 20610 with the LT and RT modifiers and then 20610-59. Medicare seems to like things like this broken out, but don't hesitate to do a corrected claim if they deny it for some reason.

I have had success with the XS applied to two of the 20610 along with RT LT and making sure the diagnosis for that joint is the only DX attached in the claim pointers. ICD-10 is so specific the DX should help you explain the X 3.
