Do I have to use code 20611 which includes ultrasound guidance or it is still ok to use 20610 and 76942 instead. Any specific guidance for New York for WC and No fault billing.? I was told for WC I have to bill 20611 but for No Fault it is ok to bill 20610 with 76942 for injection with ultrasonic guidance.

20611, per the CPT description, is the correct code if the procedure is performed with ultrasound guidance. It would be incorrect coding, and contradictory, to bill 20610 with 76942 because the description for 20610 specifically states 'without ultrasound guidance'.

Unless your payer requires it in a specific policy (and I'm not familiar with NY WC so I can't speak to that), I wouldn't use incorrect coding.
