Our doctor inject 20550 to bilateral long fingers and left small finger. Do you bill 20550x3, or 20550-50, 20550-59-F4? This is Blue Shield.

Hope this helps...assuming the doctor injected the tendon sheath.
 
I have a question along these lines, also. Our doctor inject 20550 to bilateral acromiclavicular ligaments, bilateral coracrominal ligaments, bilateral coracolavicular ligments. Do you bill 20550x3 or 20550-50? Is the code per ligament or per body location? This for BCBSTX insurance.

I can't say I've encountered this sort of situation but, going by the code description (single tendon sheath, or ligament), it does seem you would bill per ligament. Payer policies may vary but my inclination would be to bill:
20550-50 bilateral AC ligaments
20550-59-50 bilateral CA ligaments
20550-59-50 bilateral CC ligaments
