How to bill 2 visits same day-same provider for different diagnosis. Pt had puncture wound and had to return to the office later in the day for treatment

The patient returned later in the day for treatment of the puncture wound or for something else? Can you clarify if you mean the patient had two appointments that day or is your provider wanting to bill 2 E/M services?

You would combine both visits and bill based on that. Just pretend it's one visit (though the documentation should reflect the patient returned later). Both dxs would go on the same claim and you'd have 1 E&M code.

For example if the first visit was a level 3, but with the 2nd visit it brought it up to a level 4. Or if the first visit was 30 minutes time and the 2nd was 15 minutes time, bringing the total to 45 minutes which made it a level 5. Etc.
