Wiki 2 E/M Codes on one report

Hello,

Can anyone explain to me why a coder would put 2 different E/M codes on a report? Example 99203 and a 99204. I have seen it several times and wondering why you would put 2 E/M codes on an office visit.

It looks like an error by clinical provider because they are not sure. if you submit both of them it will get denied if done on same date of service. Select one of the CPTs per physician's documentation notes.
