I am being asked as to whether or not the documentation of 3 vital signs are required in order for credit in the constitutional organ system exam according to 1995 guidelines. It is clearly stated in the 1997 guidelines that it is required however I cannot find ANY documentation that is required for 1995.Can anyone please clarify?
 
There are no defined elements in 1995, so one vital sign will get you credit for constitutional. You are only required to get 3 or more to qualify for 1 of the 2 possible bullets in 97.

Thanks, Laura. Is it possible that our local Medicare carrier would have their own specific guidelines pertaining to this or is it across the board?

Is it acceptable to document 'VSS' (vital signs stable)? How many vital signs must be listed in order to receive 'credit' for the 1995 guidelines under 'constitutional'?

Answer:
The use of 'VSS' is not acceptable. The measurement of at least three of the seven vital signs must be documented.

Sitting or standing blood pressure
Supine blood pressure
Pulse rate and regularity
Respiration
Temperature

Please keep in mind the elements under constitutional may be the general appearance of the patient and/or the measurement of any three of the seven vital signs listed above
