Please help in coding op report. Not sure of cpt, either 19125 or 19301???
Procedure Performed: Left breast needle localization, lumpectomy
A curvilinear incision was made just slightly superior to the entry point of the wire, carried down through the subcutaneous tissue and then dissection proceeded on a slight angle to intercept the wire. It was noted that the wire tip was about 2.5 cm deep from the entry site of the skin. As such, dissection proceeded immediately to dissect out a circumferential lump of tissue which was approximately 3 x 3 x 3 cm, inclusive of the needle tip. The wire was transected at the skin level to allow complete encirclement of the wire as the dissection proceeded. Once the entire specimen as completely dissected, it was placed in a plastic bag and sent down to mammogram where mammogram was performed and Dr. XXXX confirmed the suspicious region to be in the specimen, central aspect. As such, bleeding points were controlled with electrocautery and the deep substance of the breast was reapporximated with 3-0 Monocryl. The subcutaneous tissue was approximated the 3-0 Monocryl and the skin was closed with a 4-0 Monocryl in a running subcuticular manner.
