I'm having one of those days... so here goes please tell me how you would code the following:

Then a 15 blade was used
to excise approximately 2 mm of skin on either side of the wound along the
dehisced incision which was approximately 6-8 cm. Once this was done, the
necrotic tissue was excised and removed. We had fresh skin edges. The wound
was then probed and at proximal aspect of the wound there was fascial
dehiscence down to the plate. There was no purulence, no fibrinous tissue
present. The wound was then irrigated copiously with 3 liters of normal
saline. It was then closed with 3-0 Vicryl in the subcutaneous tissues and 3-
0 nylon horizontal interrupted mattress sutures were placed in the skin with
I keep coming back to the 13160 :)

That's most definitely from the musculoskeletal system. Once you reach fascia, you've gone beyond the subcutaneous tissue, and it's no longer an integumentary procedure. Hope that helps! ;)

This is definitely 13160 (secondary closure, complicated wound dehisence). Procedure code 27310 is for knee arthrotomy.
