If it was a destruction of warts then you use the 17111 with one unit only coded once, this is for 15 or more and it does not matter if it is 16 or 100.
Would it be best if I tell the provider to just treat some and slap a modifier 58 when the patient comes back a week later to treat some more of them?

Not really. The only time that is appropriate is if the patient is unable to tolerate having all of them destroyed at the same encounter. It must clearly be in the best interest of the patient and not because you want to be able to bill for more.
