    Wiki Modifier with 81003 and go477

    If we are doing a Drug Screen and a Urine dip with a possible UTI or other Urine issues, Is there a modifier that can be used showing they are totally separate. We have used 59 and 91 and both were denied. thank you for any help...

    Wiki Help - physician report

    Cigna audit. We used H0003 with physician report- Cigna states we should have used G0431 we feel we have used proper code as both are basically the same. Can I get some support as they are requesting a refund unless I can substantiate. The person I used to help me with coding has passed on. thanks

    Has anyone had success with urinalysis (81002) paying separately when billing w/ a prenatal office visit to Paramount? I'm finding that most bundle. I am aware this code's fee schedule varies payer to payer. Thanks for the input!

    Wiki coding from urine culture ?

    For outpt facility clinic visits, can I code results from the urine culture? The dr saw the pt for dysuria, diagnosed uti and ordered culture, but he did not comment on the results..... The results showed E Coli.

    Wiki OB pt w/ elevated BP

    how do you code a patient how came in with elevated BP but the doctor is not calling it gestational hypertension yet. she sent the patient home to do a 24 hour urine test. should I just append the R03.0 elevated BP?

    We are getting denials for code G0477 which took place of G0434. This is the urine drug screening. Has anyone else experienced this? The dx is Z79.899

    Wiki Urine drug screens in office

    What diagnosis would we use for a urine drug screen being done for state mandated screening of patients on narcotics? We are going to bill the new code G0477 and thought about dx Z51.81, however we were unsure if that dx could only be used by labs. Any advice? Thanks, Anna Sanders Tennessee
