Wiki Sequelae of Cerebrovascular Disease

If a patient presents for a f/u to an Ischemic stroke and has complaints of muscle weakness to the Left side, am I able to assume the causal relationship considering the patient had a stroke and code a Sequelae as secondary or would the physician have to state: Left side weakness DUE TO ......."...
