    Wiki Billed Date: Date of Service or Date Read?

    Hello! I am billing for a cardiologist that reads ECHO reports in the hospital. Therefore we bill the professional component using (93306, 93308) -26. Should the billed date be the date the ECHO was performed or the date that our doctor read the ECHO? Thanks!

    Wiki billing for the read of ob ultrasound 76816

    Dr. E goes to an outreach clinic, the outreach does the 76816 TC portion on the 5th (date of the appointment). Dr. E comes back to his clinic and does the "read" on the 7th. What date is used for billing the "read" the 5th or 7th.

    Wiki 58572/58660

    When coding 58572, is lysis of adhesions bundled in this or can you also report 58660 with modifier 51? Or should 58572 be reported with modifier 22? It does not say that 58572 cannot be reported with 58660. I am just confusing myself with the more I read.....
