Wiki Carotid Endarterectomy

When performed, is a carotid angiogram included with the endarterectomy - 35301? ALSO, a separate case... carotid endarterectomy was not done due to not a lot of plaque, but the patch angioplasty and carotid angiogram were done. How would this case be coded? Thanks for any help, its appreciated!
