    Do you have to bill the prolonged service G0317 on the same claim form as your E/M visit, (99306-99310) or can this code be billed separately on it's own?

    Wiki Coding time based E/M service when time spent is less

    I've been asked by a hospitalist who sees residents in a nursing facility the best way to code his services when the 30-minute description of 99309 is more than he needs or uses. The service, advanced care planning, is accurate for his work, he just wants clarification about the time requirement.

    Wiki 99358

    We are billing 99358 to BCBS and Aetna medicare advantage plans and medicaid. They are denying this code. Is anyone else having this issue? Is there something we can be doing to get this paid? We are billing for nursing facilities. The providers review patient records before being seen in facility.
