Wiki Removal of Internal Fixation Device by another physician

How do I code for the removal of an internal fixation device on the tibial if the surgeon was NOT the one who performed the original surgery - application of the device? This is for the removal of 7 pins and a plate from the tibia. 20680 (removal of implant, deep) does not pay enough for this...
