Wiki Incident to billing to Commercial carriers

Does anyone know where I can find the commercial carriers' rules for incident to billing? I've tried searching ALL of the commercial carrier's websites and the only payer that has information is BCBS/Anthem. Does anyone have suggestions as to where to get this information? My practice...

Does anyone know if the overseeing physician has to be credentialed in the specialty of the nurse practitioner who will be seeing the patient? and if the overseeing physician can review charts digitally? And will telemedicine be acceptable for incident to billing???? If anyone can help answer...
