Can someone assist in telling me if this is a good example of co-surgeons.

in our ASC two different specialist, both did separate op notes.

Urologist did apply a sling
OB/GYN did polypectomy and myomectomy

Are we to have two separate claims, one for each provider with all same procedures and add modifier 62 to all?
or
Are we to have two separate claims, one for each provider with individual procedure they did (different) and no modifier?

If the providers are two different specialties and they dictated separate op notes for their part of the case and the procedures allow for co-surgery and they named one another as co-surgeons in each it is best to have two separate claims for each provider.
It depends on the documentation. I have seen cases where the co-surgeon only does certain parts and then leaves, so you wouldn't necessarily report all of the same CPT with a 62. Not all CPT allow co-surgery. You would only report the CPT that the co-surgery happened on. The CPT have to allow for co-surgery in the first place. Modifier 62 would go on both claims if co-surgery applies.
