Wiki former smoker vs non smoker

When can you consider a former smoker as a non smoker? My billing manager wants me to stop using Z87.891 after the pt has quit for 2 years. Not sure that this is correct.

Hard headed PA under exam he has No exam consultation only All I can do is use the 99211. I try to explain when he does that he is working for free

the HPI is expanded problem focused, the ROS is detailed. Referred out to orthopedic surgeon. It was a consult for results of an MRI not lab.
