I agree with 46250 for the hemorrhoidectomy. 46615 may be correct if they fulgrated or "destroyed" the polyp, otherwise if it was just excised it is more likley to be 46610-46612.... [ Read More ]

46250 and 46615 are not bundled per encoder. But I would probably look at how the polyp was removed because 46615 is ablation, if they excised it that's not ablation. Maybe look at 46606, 46610-12.... [ Read More ]
