In the Vol. 2, No. 10 issue of the E/M Coding Alert, the article titled “Get to Know Next Year’s Chronic Care Management Code Changes,” incorrectly included CPT® code 99498 as a new chronic care management code. New code 99498 is an advance care planning code with the descriptor: Advance care planning including the explanation and discussion of advance directives such as standard forms (with completion of such forms, when performed), by the physician or other qualified health care professional; each additional 30 minutes (List separately in addition to code for primary procedure)
CPT® 2015 will also add a second new advance care planning code the article did not mention: 99497 (Advance care planning including the explanation and discussion of advance directives such as standard forms [with completion of such forms, when performed], by the physician or other qualified health care professional; first 30 minutes, face-to-face with the patient, family member[s], and/or surrogate).
