No Real Code for Nerve Block US

Question: One of our clients is starting to use ultrasound guidance for postoperative pain control nerve blocks. Does CPT include a code for this service? Do we report it separately?

Answer: CPT does not include an ultrasound guidance code that appropriately fits needle guidance for a nerve block (there are codes for biopsy, abscess drainage and other situations, but not for guidance). If you choose to report both services, your best option for the ultrasound might be 76999 (Unlisted ultrasound procedure [e.g., diagnostic, interventional]). Remember to include any supporting documentation for the procedure if you report the unlisted code.
