Wiki Tibial Insert Help

I'm having a hard time finding a code for the surgery my provider did. He inserted a 15 mm tibial insert in to the knee of a patient that had a total knee done approximately 1 year previously. Per his note, there was a gap in the lateral and medial compartments. He also removed hypertrophic synovium/scar from about the patella. Any help I could get would be very much appreciated.

Technically, your physician did a "Revision Arthroplasty" of one component of the Knee Arthroplasty (the Tibial Insert/Spacer), even though he did not "completely replace" the entire Tibial Component (Tibial Tray). This would be 27486. Any debridement of other tissue would be an integral part of the procedure, i.e. included in 27486.
