My physician is removing instrumentation from L4-L5 and adding new instrument ion from L4-S1 so including another level can we bill for the insertion 22842 and removal 22850?
The Spinal Instrumentation guidelines say "only the appropriate insertion code should be reported when previously placed spinal instrumentation is being removed or revised during the same session where new instrumentation is inserted at levels including all or part of the previously instrumented segments". So I would say no?
What if there is an additional level, the guidelines only say all or part of the previous instrumented segments, there is an additional level, so would it be appropriate to bill for the removal.
