We billed preventative visit with V20 code and 99213 visit with a sick visit for diarrhea and vomiting. United Healthcare paid for the 99393-25 visit as child was seen on same day. United denied the 99213 as showing not a separable visit. Should I resubmit as 99213-25 and remove the 25 on the 99393 visit or leave the 25 modifier for the 99393 and just resubmit the 99213-25 visit?
 
If the documentation supports the additional E/M visit, the 25 will go on the office visit (99213). If there were any other procedures done that day (injections, etc.) you may need to keep the 25 on the the 99393. If not, it would not be necessary.
