Can someone please help me with this chart?

PRE-PROCEDURE DIAGNOSIS:
Pelvic pain, pelvic venous congestion syndrome
POST-PROCEDURE DIAGNOSIS:
Same
PROCEDURES PERFORMED:
INFERIOR VENACAVOGRAM, LEFT RENAL VENOGRAM, LEFT AND RIGHT OVARIAN
VENOGRAMS.
IMAGING MODALITY UTILIZED:
Ultrasound and fluoroscopy
ACCESS SITE:
Right common femoral vein
CATHETER POSITION:
Inferior vena cava, left renal vein, left ovarian vein, right ovarian vein
CONTRAST UTILIZED: Nonionic contrast utilized.
TECHNIQUE: The skin overlying the right groin was sterilely prepped and draped in
standard fashion as described above. Under ultrasound guidance after
achieving local anesthesia with 1% lidocaine, the right common femoral vein
was accessed. Over a guidewire a 6 French sheath was inserted, followed by
placement of a 5 French C2 glide catheter into the left renal vein.
Injection performed. The catheter further advanced into the left ovarian
vein. Injection performed. Catheter exchanged over a guidewire for a 5
French Reuter catheter which was placed in the right ovarian vein.
Injection performed. Catheter exchanged for a 4 French Omni Flush catheter
placed in the caudal inferior vena cava, inferior venacavagram performed.
Catheter removed. Sheath removed. Hemostasis achieved with compression.
3000 units of heparin were administered for the procedure.

My thoughts are to use 36011 for the cath placement in the right ovarian vein, 36012 for cath placement in the left ovarian vein, and 75825 for the IVC. I'm not sure what to use for the venographies of the ovarian veins. Thanks for the help!
