Pt has AVF with the Radial an cephalic. Both of these are angioplastied and also the perforator vein is angioplastied. Is this coded separate or the perforator vein considered part of the dialysis circuit? All done thru thru the same access.

I think it would still only be 36902. The book I have says 36902 includes angioplasty of one or more lesions of the peripheral segment of the dialysis circuit. The peripheral segment begins at the arterial anastomosis and extends to the central segment and in the case of a radiocephalic fistula, the peripheral segment extends through the entire cephalic vein.
