SURGEON: MY PROVIDER

PREOPERATIVE DIAGNOSIS: Left adnexal mass.

POSTOPERATIVE DIAGNOSIS: Cystic left adnexal mass.

PROCEDURE: Lysis of adhesions and removal of left adnexal mass.

ASSISTANT: Dr. ?

DESCRIPTION OF PROCEDURE: Dr. ? is the attending surgeon of record, and he put his trocars in
and took out the patient's gallbladder and did an extensive lysis of adhesions, taking down the
omentum from the abdominal wall, and the pelvis. There was no free fluid, no excrescences, no
evidence of malignancy. He put in a larger hand port, and we were able to free up the remainder of
the adhesions in the pelvis and identify the left adnexal cystic structure. I put my hand in. I
freed it inferiorly, laterally, and posteriorly and brought it up, and Dr. ? used the small
LigaSure across the blood supply to the mass that was probably her left ovary or cyst of the left
ovary. Could not identify tissue that looked ovarian on the right or the left side.
The right ovary could not be identified.

The mass was sent down for frozen section. At this point, Dr. ? took back over and closed his
ports, and the patient will go to the recovery room when he has finished his portion of the case

PER PATHOLOGY REPORT NO SIMPLE OF OVARY WAS TAKEN. ONLY TUBE & MASS
