Hi Coders! I need help coding a hip surgery. The patient had a prior femur nail for a fracture. This failed, and the doc planned a hip revision to a total hip arthroplasty - I got the coding for that (27132). During the surgery, it was found that almost the entire femur had deteriorated from avascular necrosis to the very distal end of the shaft. Doc had to use a prosthetic replacement for the missing bone. He spent an extra 2 hours (over his normal 3) on this procedure. I am at a loss as to how to accurately capture the work for the femur excision and replacement. Any help would be greatly appreciated.
