I'm located in Florida (First Coast Service Options region), is anyone else have a ton of issues suddenly this year with Medicare Secondary Payer denying all claims for CO-16/N245 denials saying something is missing about the primary insurance? We are submitting claims exactly as we always have done but as of 2022 all claims are rejecting for that reason. I've tried calling Medicare and they are not helpful about what's wrong. It's not clearinghouse-specific either, I'm having this issue across multiple provider offices/clearinghouses so I'm assuming it's something that changed in how MSP processes claims this year.
