when reporting the functional limitation g codes do i put the severity modifier first then the gp modifier or do i report the gp modifier then the severity modifier? Help!

We have not been using the "GP" modifiers on the funtional status HCPCS codes; have only been using the severity modifiers, and have not had any problems doing it this way. Our MAC is Novitas.

I am not sure if the procedure varies by MAC but with Noridian we are still required to report GP or GO modifiers on the claim first then report the severity modifiers 2nd but do not use any other mods such as 59 or KX etc.
