If Myelofibrosis D75.81 is the only condition documented, and a query was sent to find out the underlying cause (as necessary since this is a manifestation code) but no response from provider, would you still send out with the D75.81?

Since the D75.81 includes 'myelofibrosis NOS' under the descriptor in the Tabular List, I would go ahead and code it as primary even though there is a 'code first' note. It's possible that a payer may reject it for incorrect coding, in which case you may need to try another query or escalate the issue to your manager for resolution.
